   * What led up to the situation?

- upgrading virtualbox to current version without also upgrading extension package

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

ineffective:
- click "start" on a VM
- watch it beginning to boot
- see the VM's window vanish
- see the VM's state is now "Aborted"

effective:
- find "segfault", "error 4 in VBoxDD.so" in dmesg
- google
- find solution

   * What was the outcome of this action?

- VMs don't boot as expected

   * What outcome did you expect instead?

- an useful error message about needing to upgrade or remove the
outdated extension package

I agree that a check for the extension against the version of VBox would
be a good thing. But that's something I'd defer to the upstream devs. I
really don't have the time/enthusiasm to do it. If you really care about
it, please file a bug report upstream.
