IDA Plug-in writing tutorial (v1.1)

Hi all,

For those who might be interested, I've updated the IDA Plug-in
Writing Tutorial to cover IDA Pro 5.4. The SDK was mostly frozen as
of 4.9, although there have been some subtle changes.

All in all there are about 10 new pages including some corrections,
new functions and a new sample plug-in. Get it here:
